About cab cars: Essentially all of the civilized world is operating push-pull trains or E/DMUs. However, the civilized world has crashworthyness standards which go beyond buff strength. They use energy absorbing elements (which may make the vehicles look somewhat funny). On impact, they crumple, and the cab may look munched up, but when well-designed, the space where the driver sits is unharmed.
So, absolutely viable…
About seat belts: who would be using seatbelts? In Europe, buses now must be equipped with seat belts… but you can count the passengers wearing them on an amputated finger…
Grade separating… Would the towns be willing to pay for grade separation? Would you be willing to pay higher taxes for grade separation? On the other hand, would you be willing to drive 200 meters farther, because "your" grade crossing has been closed?
OTOH, one question has not been asked: why did three cars not stay upright?
