I don't want to discount what you went through, because I wasn't there. However, if you were on a train that was about to run through a gang, the failure most certainly didn't occur solely because someone wasn't looking out the window.
Again, I refer you to my fog (low visibilty) analogy.
Unless operating under restricted speed, one could run an entire subdivision without windows with working PTC.
