Interesting and concerning reading. A concerning trend, though, is the tendency to assume that drugs in someone's system was the cause of an accident. I don't think the NTSB goes that far but many investigators do. The drugs may, or may not, have been a contributing factor. Unfortunately in the case of the two deceased workers we'll never really know.
