One take-away, the engineer and both maintenance workers tested positive for drugs. And for how many years have they been hauling off employees for random drug tests, and this is still continuing. If this is the best solution, then maybe not wasting time testing purchasing, finance, and other behind the scenes employees would be a good start (though some decisions out of them certainly at times seem like there could be drugs involved!!), and focusing more on the front line employees. But it needs to come with a focus on rehab and not so much "punishment." Management and Union share this.
An organization has to want to change, otherwise these chain of events (series of failures) that lead to the accident, will continue to occur.
