I understand the engineer was in violation of the rules, but that is no consolation for passengers badly injured or dead, and a bunch of torn up equipment.
Obviously the rules haven't been enough to prevent wrecks. I think the industry needs to recognize that human error is inevitable, and take steps to make really big mistakes less likely. Lit-up signs at the approach to slow curves would help, and would be a lot cheaper than PTC.
Aircraft have stall warnings and terrain proximity warnings which are needed only when the crew screws up. The US airline industry averages 87,000 flights per day and hasn't had a fatal crash in the last 7 years.
