As has been mentioned before, people should be be allowed to "speculate" on the cause of this crash in a forum like this, lacking any further information. I, personally, found the article to be well composed and probably coming as close to describing the circumstances and likely contributing factors as anything else we have.

Having found myself in a state of momentary "loss of situational awareness" due to unfamiliarity with equipment and territory I have an appreciation for how this can happen, and how scary it is to feel yourself in that situation, sometimes only realizing it after it has happened. Sure "sounds" like the qualifying procedures for opening this line were rushed, but the final determination on this will have to come from the final NTSB report.
