Some basic questions -- some of which are a matter of basic facts, others of which will require investigation:
Is this section of line double track? (Could one of the trains have been on the wrong track?)
Does this section of track have a block signal system? (Dis one of the trains run a signal?)
What kind of procedure is used to inform revenue trains of work on the line?
Was the work train adequately illuminated to call attention to itself? (One would think work trains would have strobe lights or other equipment to make them move visible.)
At the very least, this appears to involve some type of dispatching error.
But, as is typical in these types of accidents, there were probably a combination of factors that contributed.
