There's so many bridges, highway and rail that fall into this... here's a few I can think of....
BN/NP capitol lake Olympia. Drawspan, moved from another location. Never intended to work.
Several drawbridges on the Sloughs near Everett, WA. Some used, some used rarely, some no longer.
In the Wilamette Valley, in Salem there is the former SP draw. Albany has another former SP Draw. Corvallis has a road draw, not used since the 30's. Further south on the OE and SP line near junction city are two more lift spans over the Wilamette river, probably never used.
Most famously, there are a pair of draw bridges over the Missouri river in Montana and North Dakota, the Snowden and Fairview bridges. You can walk across Fairview, Snowden is still in rail use, but I don't believe either of them ever opened for river traffic apart from their initial test lifts.
