The railroads in that region burned a byproduct of the local coal that required these huge fireboxes that spread out far beyond the axle width. The cabs were put in front of the firebox as there was no room for them over it.

Because this put the engineer right over the driving rods, it was a very unsafe design, and was eventually made illegal (for new construction).
