I would answer generally no, in some cases maybe you would at low speed on a yard track, but not necessarily in most cases. There's a lot of motion and other track noise, so slicing through the points at speed wouldn't stand out... Which is probably why we had to take that eye test to drive the choo-choos, too...
When you are coming to a signalized junction, at the end of two track operation, running a passenger train, still accelerating from the station stop, and you pay no attention to the signal or the switch position, because you are too busy texting, you are distracted. This is just the same as the idiot who ploughs through a crosswalk full of school kids, when driving while texting. So no, he wasn't overworked. He was known by fellow crewmenbers as a pretty scary engineer, who'd done scary crap like this before, and who'd even been turned in by fellow crew members to Veolia, who did nothing..... So, I'm gonna go with the NTSB report, instead of those still making excuses...
