Don't forget:
Some contractors (or their subs) intentionally underbid the job to get the work, they cry poor that they can't do it for their bid and ask to renegotiate. I heard that Herzog was famous for doing this on commuter operating/maintenance contracts.
Some companies intentionally bid at or near break even expecting that the inevitable delays or change-orders will give them their opportunity to make an often healthy profit. If it's a design/build or cost-plus contract even more opportunity.
It's more likely the incompetence of one of more contractors, suppliers, design engineers/consultants and/or politicians - or sometimes $hit just happens.
