For short distances Lufthansa through tickets you on a DB ICE. The classic example is from Frankfurt Flughafen to Köln. Your Lufthansa ticket has a rail section. When I travel from San Francisco to Cologne that is the only way to fly. Before 9/11 they used to check bags through to Cologne's train station. Now you pick up your bags just before the entrance to the long distance train station at the airport, which saves having to carry them from baggage claim. Travel time from the airport to downtown Cologne just across from the cathedral is 57 minutes.
There is also a local train station at the airport if you are just going into Frankfurt or the suburbs. And yes, you get frequent flier miles for the train on a Lufthansa ticket.
