Regarding the New York subway:

According to this very informative official MTA video (2:53 long) the conductor makes a visual check to see that no objects or people are stuck in the train doors. But I agree with Commenter that a dog leash would be almost invisible several hundred feet away, and this is a very unusual (freak) accident it seems.
