We've cut accidents way back, and there are many new tecnologies that have helped improve the odds of "surviving" a trip on transit (like all of the in-vehicle and in-station cameras, that the NTSB will be combing through to see what happened).
It just seems that folks are able to find all kinds of clever ways to do very dumb things, and we just don't yet have a way detect that, yet....

We'll see...? But, my experience of the use of camera's on transit which have recorded unfortunate events like this, is that they make it harder for ambulance chasers to collect, not easier.
