Just stop running trains and that will end all incidents.

Remember the NTSB is a Government body with a mission. They are not concerned with Making a profit or if any of its recommendations meet any reasonable cost-benefit analysis. If they did, they would never have recommended PTC as it doesn’t meet any reasonable standard.
Every Govt Dept is constantly trying to justify their budget & expand their domain. It’s the nature of Bureaucrats to keep trying to show you just how important they are lest you discover that the world wouldn’t be any worse off without them. It gets worse as the “low hanging fruit” has already been picked so most safety improvement is minimal at best. Higher Cost & Lower Return for each incremental step as most Engineers & Economists are well aware of. Bureaucracies expand as results diminish.
