California Public Utilities Commission General Order #143B requires turn signals on light rail vehicles built after 1956. In all of the systems I'm familiar with, they're activated by the operator. In Sacramento's case, they also activate the gong on each end of each car in the consist.

The San Francisco cable cars and historic streetcars don't have them, nor are they required to.
