I can't speak for UP; but I'm pretty sure a so-called high-cube container double stacked on the BNSF does not get special annotation on BNSF wheel reports...a 'High-Wide' load is coverd under a Form C instruction...on the Mojave Sub on the UPRR (and perhaps Roseville, too) the detectors do not have a recall feature, so if you miss the detector announcement, you're out of luck, unless it 'trips' the detector, then it gives a preliminary warning followed by axle count location and a repeat of the defect once the train passes the detector. Published instructions govern corrective action.
No amount of fail-safe products ever devised can counteract gross negligence or indifference. It all starts with a thorough job briefing.
