A false flange is a "flange" on the wrong side of the rim of the wheel. It can be caused by sliding the wheel and making a very large flat spot or an extreme amount of wear, in which case it's also referred to as a hollow tread (or hollow wheel). In either case, the false flange derails on specialwork like switch frogs and switch points because rather than riding up gradually over the wing and stock rails, it shoves them sideways, allowing the wheel to drop down onto the ties.
