Did they bother to ask anyone that had prior experience in such matters before taking over?
This epitomizes most of what I see on a recurring basis where I work. Little to no effort is made to retain experience, nor truly set a standard practice away from past mistakes.
Old heads with 40 years of experience are often given a supermarket sheet cake, a piece of crap watch made in China, and shown the door with us younger guys screaming "seniority move!"
Sometimes, rules are changed after a fatality, but little explanation or follow-up is made amongst the troops as to why.
Anyway, great to hear the UP is taking it seriously enough not to scrap the machines that kept it open for so many years.
