In one case an operator was leaning out the window when the train left 19th Street station in Oakland and re-entered the subway. There is very little clearance between the train and the subway tunnel wall, and he was hit and suffered a severe head injury.

The train automatically went to 12th Street and stopped with no assistance from the operator. It probably would have continued past 12th Street but onlookers had seen the accident and alerted BART.
