The picture looks like what happens to rail on the inside of a curve. It happens from the inside wheel when having to compensate for the fact that the outside wheel is traveling a longer distance. That is the reason curves are laid to a wider gauge. The slope of the wheel is how historically they compensated for this. But with concrete ties this becauses a problem. Because gauge is all the same in curves and tangents. Not saying I know for sure that is what is pictured. Because that is worst then I have ever seen. But that is my opinion.
