The comment about there being a double-slip in the entrance to the Caltrain station in SF got me looking. The previous SF Google was a little off - it was on the light rail line, and definitely was just a double crossover. Scroll a little bit north, to the Caltrain tracks, and you find this which definitely *is* a slip - can't see enough detail to be sure it's a double (might be single).
