There is; follow the links to SPAD episodes. Although I couldn't find statistics for the same type of violation in the US, the number of similar incidents in the UK seems a bit high for a country so small geographically. There was a big scandal about signal reliability right after BR qwas privatized, so I get the impression the contract operator of these BR lines (which permits the preserved engines to be operated by the societies) is trying to avoid a Chatsworth-like incident. It would be most illuminating to see whether the same standards and requirements are being applied to the operator's own employees. It could also be a move similar to what the UP is doing to private cars on Amtrak trains: trying to kill them off.
