First question? Why was an industrial spur on a major passenger line not protected by a derail on a grade where a car could "roll out" onto the mainline? Second question? The set out car should have had both air and handbrakes set? ---- Although it is not uncommon for customers to bleed off the air, and release the hand brake to reposition the car for ease of unloading.
Reminds me of the unfortunate Everett Jennings incident in Los Angeles on the West Hollywood branch in the seventies? Thank god no one was killed in this incident.
