Another theory- sudden acceleration

With all the new "vehicles" running everything off "Computer Controlled Fiber" in place of linkages gears and cables and without warning going full throttle (like Toyota's) could have been a cause why the locomotive increased speed and the engineer spent time trying to get the speed under control instead of plugging it right away.
