They had a pilot and co-pilot at Tenerife....?

I've seen plenty of the same sort of accident with two or even three folks in the cab...?

Clyde Patterson's theory in his article about the wreck at Santa Anita, I think shows what often happens, that, like with the aviation disaster at Tenerife, those who might have spoken up, like a fireman or co-pilot, are often intimidated by those in authority, and don't act.

I have also seen where someone made an error and where it was caught and corrected by a second set of eyes. So it may have helped, in a case like this, but is definitely not the "magic bullet" which can prevent this kind of human error.

One reason I took the buy out, was when, as a union officer, I realized that folks like Byron Boyd and Charlie Little would gladly sell out the men (and women), on everything from the crew size, to killing the five day switching jobs, and allowing conductors to operate remotes. So that ship has sailed, and as noted, that is not sufficient to ensure a higher level of safety.

The computer, as far as we know now, didn't likely cause this accident, but a computer with the speeds and location being monitored correctly, would almost certainly have recognized this human "error", set the air, and prevented this accident from happening.
