Rusty wheels and rail or defective rail motion sensors are not 100% effective. Many railroads now require any movement consisting of less than 12 axles must be prepared to stop short of any crossing where the signals are not functioning. Then they may flag their way over the crossing after ALL vehicles have stopped.

Bottom line, always look both ways before crossing tracks even if it is allegedly protected by crossing signals.
