last guy running

I had worked for a shoreline and if you created flat wheels or other defects they always knew who the last guy running was. I always wondered on the class 1 railroads if engineers would take responsibility for creating flat wheels or other error caused defects when they could just pass it on to the next crew. Is this common? Do certain engineers have a reputation for passing on defects they created? I guess with all the event recorder information now days it's harder to deny it.
