Back in '82 while I was sitting the Airbrake portion of my Enginedriver Certificate, our tutor handed out several NTSB Reports during discussion about taking shortcuts under pressure of time or weather.
This one stuck with me through the years.

Oddly years later an ex-BN Engineer I met in Colorado mentioned he was on the wrecktrain follow up.
