Why Does The News Media Have Such a Hard Time Reporting Something Accurately?

How many times have we read or heard a railroad-related story and they get the roles of the engineer and conductor inverted?

Instead of calling BNSF or CSX a "railroad", why do they call it a "train company"? Do they refer to Delta or Southwest as "plane companies"?

Why do they get "cars" and "trains" mixed up? Instead of saying, "Fifteen cars derailed at East Palestine", they'll say "Fifteen trains derailed at East Palestine".
