This Washington, D.C. Metro Crash has similarities to the November 2007 Chicago NS Freight Train vs. Amtrak Crash as it was a case of a train being rammed from behind when it was stopped like what the Amtrak Lake Shore Limited Train did in a freight yard in Chicago when it rammed the back of a Norfolk Southern Freight Train.
What is usually the cause of such mishaps? Non-attention by the engineer to the signal indicators maybe? Or just faulty signaling? Or both?
Do most modern subway systems in the United States rely on computerized dispatching rather then on in-person dispatcher aided dispatching? Or the opposite? I though BART was an example at one time of heavy use of computers to aid in dispatching?
