I can tell you with reasonable certainty that condition is most often caused by slowly trying to squeak by a car or engine that is not quite in the clear- only hoping it is. The results are usually a torn off visor. The key thing that is occasionally overlooked (aside from NOT leaving cars in the foul!) is to pull in the visor.
