I seem to recall that de-stranding of the cable caused a very serious accident in the Chicago Cable system leading to its early demise. Of course, in those days, there was no way for the crew to have the cable shut down. A car was carried along without ability to stop until it had overrun many other vehicles and piled up at the entrance to a tunnel. They used a different kind of grip than the SF cars (side grip rather than top grip).
