Yes. UP, BNSF and Amtrak have them. Metrolink has just started. The Napa Wine Train was an early user. I'm sure there are plenty of others, particularly Class I's.

Now that, I don't know. The ones I'm familiar with aren't horn activated.

I'm not aware of any collective bargaining agreement that prevents it. The BLET just lost a suit it filed against Metrolink, which is installing both forward facing and cab interior cameras.
