Runaway rail car

Perhaps I missed it, but seems that nothing is said in the article concerning why the brakes weren't set on the car. Isn't it kinduva standard procedure to hand-crank the brakes on standing cars, or at least on one or both of the end cars in a string of them? Or am I too optimistic about that being routinely done these days?
