The essential problem with Caltrain and 4th & Townsend is that Caltrain riders do not vote in San Francisco, so Muni cares little about them. As far as I know, there is, as someone mentioned, a ticket dispenser on the N and T platform, and, failing that, you can enter at the front and give the driver a cash fare the old-fashioned way.
A day pass is another matter. As someone pointed out, there are no human ticket vendors at Caltrain any more, but it is hard to understand why Muni could not deputize the coffee bar to sell day passes.
