From my limited knowledge and for earlier days than roller bearings the crew was expected to do whatever was needed to get the engine to a terminal if at all possible. "The Steam Locomotive Up-to-Date" (1899) has lots to say about temporary repairs on the road.

When 6315 broke down on a fan trip in 1959, the crew handled it. We were luckey enough to have a second engine (5632).
