Article I saw said the driver went to fix a glitchy door - from the outside - without setting the brake or otherwise disabling the train. So when the door closed the automatic train control (which is active in the subway) said to itself: Great; let's go. Leaving the driver standing on the platform. I didn't know the drivers were surplus in the tunnel...
