If we are talking traditional railroading - then you do need to consider how to provide protection against following movements using TO&TT. I suppose in ABS and Yard Limits, flag protection to the rear wouldn't be an absolute necessity, but in dark territory you would need a flagman on the rear.

Presuming you are on single main track, then you would also need a means of issuing additional train orders enroute. If you aren't going to use radios, then you need operators for train order stations, or you could pay the crew extra to copy them.

There was nothing wrong with TO&TT, it's just that radio communication and CTC allows a dispatcher to have more control (good or bad) over train movements. Unfortunately this has gone to the extreme and become totally dependent upon prompt and consistent communication with a competent dispatcher - and soon how to keep the PTC happy.
