The only time I have ever seen what you refer to as a "False flange" is when a traction motor has siezed up and locked the wheel where it could not rotate. If un detected it will wear the wheel tread off and the flange will contact the track spikes and the unit will derail on the nearest road crossing or switch.
I have observed on many occasions hogheads inadvertently select reverse with the trottle in foreward configuration and have the fuses on the traction motors blow freeing the motors.
How can you use the reverser to freeze the traction motors?
