Really? I mean, come on, really? Since you spent most of your career in the Engineering Department, how "many occasions" did you actually see this happen, especially since the throttle and reverser are interlocked?

How? By following the steps in the thread below that describes how to do it. You probably won't get every axle to seize and it won't work if the train is running away at high speed, but at lower speeds, I'd sure give it a shot. It's called "slugging" or "slugging it." Frank Holland testified at the NTSB hearing that he'd never hear of it, and Gary McClain testified that it wasn't taught at the simulator at that time. My main concern would be for a flash-over inside the electrical cabinet and a cabinet door blowing off.
