Both the Pittsburgh Siemens cars and the St. Louis Siemens cars have foot pedal control. They were specified by Pittsburgh and were included on the St. Louis order as those cars are a derivative of the Pittsburgh car. I'm pretty sure that the CAF cars in Pittsburgh are also equipped with foot controls. The CLRV (4 axle) and ALRVs (6 axle) in Toronto also have foot control. There are no longer any patent issues with foot controlled cars that I'm aware of.

Most, but not all PCCs had three pedal control, with the left pedal being the "deadman", the center pedal controlling the brakes and the right pedal controlling the rate of acceleration.

Cars built for St. Louis Public Service had left foot accelerators, right foot brakes and the "deadman" in the heel!

The Chicago Transit Authority PCC streetcars had "bicycle handles" for controllers.
