After they gotstopped the first time, the train should not have been moved again until a thorough inspection and air brake test had been made, holding it with handbrakes while this is done. This would include testing the FRED tomake certain it dumped on demand.

Even then, they should have thought about taking the cars into the yard in cuts, rather than
turning the whole thing loose again.

Plenty of blame to go around.
