This relatively minor accidental collision from making a mistaken turn (which has happened repeatedly at this confusing intersection without this level of damage and without regard to the race of the driver) was not the actual cause the catastrophic derailment and "massive" damage, nor did the impact with this light truck and small trailer kill the engineer.

A major design failure in the Rotem ("safety") car caused the resulting catastrophic derailment (and "spin out" after the initial actual impact) and almost all of the resulting the "massive" damage, from what would have been an easily survivable impact (even with an "unsafe" Bombardier cab unit), which resulted in the death of the engineer.
