or were relieved before the replacement crew showed up, and that crew didn't go back far enough to start finding the set brakes before they started moving the train.

It appears there is a need for a log book, that stays with the locomotive, to tie-in the relieving crew as to the condition of the train. I have no experience in the trade to know if such practices exist. Poor communication seems quite often to be the culprit in many types of accidents or errors and not just the railroad or transportation industries.
