What caused this derailment???

On the following website [www.youtube.com] when the search term "BNSF" is entered there is a video that can be viewed online on this website taken from a camera placed in the cab of a BNSF Dash 9 locomotive that shows it getting into a head on side swipe collision with another BNSF frieght train as it attempts to enter the siding but the on coming BNSF freight train dosen't stop to allow it to move into the siding safely and then the accident occurs. A crew member can even be observed on the video jumping from the oncoming freight train to safety.

Does anybody know where and when this accident happened and what the aftermath of the consequences were from this derailment?
