BNSF seems to have repetitive problems with this type of incident. A few years ago, a BNSF crew left open a switch that caused a DM&E train to enter a yard and crash into parked cars. One or two DM&E crewmen were killed in that incident.
Also, TWO separate derailments at Bieber on the Gateway Subdivision have occurred. Once, the north switch was left open causing a southbound train to derail and crash into a building, and later the south switch was left open causing a northbound train to enter the yard and derail. I can't remember now if the northbound train also hit parked cars or not.
