It is interesting that they did not describe the type of signal installations involved, I would assume that they were all search light type signals, as colored position signals used by the UP, and not by the merger RRs would have also eliminated the possibility of this accident as the engineer would only need to know if the light was high or low on the mast.

Also they did not discuss 'advance approach' which would be flashing. I wonder if the engineer not seeing the signals was not the true cause, but maybe some medical condition where he did see the indications and did not react accordingly.
