I don't have the data to prove it, but I'm going to go out on a limb and suggest that nearly all fatalities on bicycles occur on streets. I would guess that the number of fatalities on bicycle trails is very limited.

Where I live, a bicycle path parallels a relatively busy CSX line. The path crosses the railroad at one location, but I am not aware of any bicycle-train fatalities at this crossing.
