Not sure what the particular rules are in each state, but it isn't uncommon to find crossings that the bells only ring while the gates are activated and coming down, and sometimes coming up. The option has been in standard signal wiring diagrams for years, and most gate mechanisms I've seen have a cam for that. IIRC, most of the San Diego Trolley crossings the bells only ring while the gates are coming up and down.

By contrast, ODOT wants two bells on any crossings where pedestrians may be around. Not sure what they want for bells if gates are down.
